Nociceptive pain arises from various kinds of trouble in tissues. This is the type of pain everyone is most familiar with from repetitive strain, injury to muscle spasms which are the leading cause of back pain. Back pain is divided into three categories, middle back (thoracic), lower back (lumbar) and coccydynia (tailbone or sacral) based on the segment affected.
Common underlying causes of back pain include degenerative or traumatic changes to the discs and facets joints, which can then cause secondary pain in the muscles, and nerves, and referred pain to the bones, joints and extremities. There are many causes of back pain but 90% of people with back pain are diagnosed with nonspecific acute back pain in which there is no serious underlying pathology.
About 9 out of 10 adults have back pain at some point and 5 out of 10 experience recurring pain annually. For most individuals, acute lower back pain is self-limiting, causing 40% of missed work and is the leading cause of disability worldwide.
The structures of the human neck are anatomically grouped into four compartments; vertebral, visceral and two vascular compartments. Within these compartments, the neck houses the cervical vertebrae and cervical part of the spinal cord, upper parts of the respiratory and digestive tracts, endocrine glands, nerves, arteries and veins. Muscles of the neck are described separately from the compartments.
Neck pain may arise due to muscular tightness in both the neck and upper back, or pinching of the nerves emanating from the cervical vertebrae. Joint disruption in the neck creates pain, as does joint disruption in the upper back. If this support system is affected adversely, then the muscles in the area will tighten, leading to neck pain.
Fifty percent of neck pain episodes resolve within a year and only 10% become chronic. Today neck pain affects about 330 million people globally.
The knee joint consists of an articulation between four bones: the femur, tibia, fibula and patella and they are divided into four compartments, medial and lateral tibiofemoral, the patellofemoral compartments and the superior tibiofibular joint.
All of these joints can suffer from repetitive strain, injury, syndromes or disease. Knee pain can be attributed to many different causes. High Impact movements on the knee joints; lower level of physical activity and degree of physical movement tends to weaken the knee muscles and age progression of the knee joint involves higher friction with adjacent tissue and cartilages.
About 25% of people over the age of 50 experience knee pain and more than $3 billion dollars is spent annually on arthroscopic knee surgeries in the US that are known to be ineffective in people with degenerative knee pain.
The shoulder joint is composed of three bones: the clavicle, the scapula, and the humerus. The AC joint located between the acromion and the clavicle facilitates movement. The shoulder joint is a ball-and-socket joint that allows the arm to rotate in a circular fashion or hinge out and up. The shoulder is the most movable but unstable joint in the body because of the range of motion allowed. This instability increases the likelihood of joint injury, often leading to a degenerative process in which tissues break down and no longer function well.
Shoulder pain may be localized or may be referred to areas around the shoulder or down the arm. Dislocation, Separation, Fracture, Degenerative Disease are all factors in determining where the pain stems from.
The most common reasons for physician visits for musculoskeletal symptoms is shoulder pain. According to the American Academy of Orthopedic Surgeons shoulder pain has been rising with over 13 million patients of which 34% were related to injury.
